PACES Student Agreement Form The Pathways to Academic and Career Exploration to Success (PACES) program provides support for adolescents in their successful transition to and retention in post-secondary education, career technical education and workforce readiness. While working with their assigned PACES Coach, high school students’ academic needs will be addressed, assistance with college and job applications will occur, and workforce development will be explored for those not interested in postsecondary options. The PACES Coach will provide an overall support system, and will collaborate with CP&P (if the case is open), transitional living or independent living providers, resource families or other supportive adults, as identified by the student/adolescent. Students/adolescents will also be connected to appropriate regional education and employment resources, including tutoring centers, career centers, on-campus resources or other resources as needed. Student/adolescent’s involvement will remain confidential, and updates will only be shared with support team or with permission from student/adolescent to external entities. If involved in the PACES program, the student/adolescent must agree to the following: · Meeting regularly with their assigned PACES Coach o In-person meetings must occur at least twice a month OR bi-weekly, as part of the program’s requirements (exceptions can be made for out-of-state students) o Students who lack participation with PACES, such as not scheduling a meeting with their PACES Coach in a period of 60 days, will lose services · Responding to PACES Coach outreach and questions o Students/adolescents who do not respond to calls, emails or texts from their over a period of 60 days, will lose PACES services o It is recommended for students to respond within 48 hours of PACES Coach contact · Completing assignments or directives from PACES Coach in order to enhance independent work and follow up · Updating PACES Coach of changed address or contact information (ie: phone number, email address, change in placement, etc.) · Actively engaging with and working with their PACES Coach as they complete skill-building activities, applications and other exploratory practices to enhance their education and employment outcomes o The PACES Program is a youth driven program, and is meant for all adolescents to write the next chapter of their lives, and to be an active participant and decision-maker in the process of academic and employment goals · Notifying PACES Coach of serious academic changes (poor grades, dropped classes, academic dismissal, financial aid verification) or employment changes (loss of position or behavioral conduct) for Coach to assist · Notifying PACES Coach of approaching deadlines for college and/or job applications, examinations, course requirements or other topics where immediate assistance may be required I have read and agree to all rules and disclaimers listed above.* I have read and agree to all rules and disclaimers listed below.The Pathways to Academic and Career Exploration to Success (PACES) program provides support for adolescents in their successful transition to and retention in post-secondary education, career technical education and workforce readiness.
